PERSONAL DATA PROTECTION LAW NO. 6698
RELATED PERSON APPLICATION FORM WITHIN THE SCOPE
Personal data owners defined as "relevant persons" (referred to as "Applicants ") in the Personal Data Protection Law No. 6698 (referred to as " KVKK ") are granted the following rights within the scope of Article 11 of the Personal Data Protection Law:
- Learning whether your personal data is being processed or not,
- Requesting information if your personal data has been processed,
- Learning the purpose of processing your personal data and whether they are used for their intended purpose,
- Knowing the third parties to whom your personal data is transferred at home or abroad,
- Requesting correction of your personal data if it has been processed incorrectly or incompletely, and requesting that the action taken in this context be notified to third parties to whom your personal data has been transferred,
- Requesting the deletion or destruction of your personal data in case the reasons requiring processing are eliminated, even though it has been processed in accordance with KVKK and other laws and other legislation provisions, and requesting that the transaction carried out in this context be notified to third parties to whom your personal data has been transferred,
- Object to the emergence of a result against you by analyzing the processed data exclusively through automatic systems,
- Request compensation for the damage if you suffer damage due to unlawful processing of your personal data.

This application form has been prepared to determine your relationship with our Company in order to evaluate and finalize your application, to respond to the requests in your application, to identify your personal data as the relevant person and to respond to your relevant application correctly and within the legal period.
Our company reserves the right to request additional information and documents to determine identity and authority within the scope of the application and to evaluate the application. The information and documents regarding your requests submitted within the scope of your application must be submitted by the authorized person and must be accurate and up-to-date. If an unauthorized application is made and it is determined that the information is not accurate and up-to-date, our Company will not take responsibility and in this case, our Company reserves the right to reject the application.
The information and documents specified in this form and transmitted to us will be processed by our Company limited to the purposes of evaluating, responding to and concluding the application made in accordance with Article 13 of the Law.
The information obtained within the scope of this form and application can be collected in writing, verbally, electronically or physically. In line with the investigation carried out within this scope, the relevant information may be shared with the company and its subsidiaries and with third parties and companies from which services are received, such as law offices and consultancy companies, in order to finalize the application in question. You can exercise your rights under Article 11 of Law No. 6698 in the procedures and conditions specified in this form.
